What is your suggestion?: A twisted league game mode like in OSRS but permanent. Without being able to 

Is this in OSRS?: Yes, but not permanent

Has this suggestion been accepted already?: No

How would this benefit Alora?:It will attract people that enjoy the twisted league game mode and the buffs you can give to an account to  Alora.

We already have something like this being planned, has been an idea ever since the first PvM League that got hosted. ;)
